New York, Form POA-1
Fax is preferred at 518-435-8617. ET-14 uses 518-435-8406. New York does not accept IRS Form 2848 or NYC Form POA-2 for this unit. Do not mail a copy of the same form you fax.

What New York wants on the form:
- A person, not a firm. Example: Alex Counsel, New York CPA, with a PTIN such as P01234567, or an SSN or EIN, plus their NYTPRIN if they have one.
- At least one tax. Example: personal income tax. If that section is left blank, New York treats the form as covering every tax and every period.
- A real period. Example: 2024, or 3/1/2024–2/28/2025 if it is not a calendar year. Sales tax can use the date of the transaction. Do not write “all open periods”, “current”, or “collection matters”.
Illinois, Form IL-2848

Florida, Form DR-835
850-488-5997 is the reemployment-tax power of attorney line for DR-835, not a fax number for every Florida tax. Confirm the form name before you send. No Florida wizard on this page. Use generic send.
