Nine digits. Example: 123-45-6789.

Your mailing state. You can live outside New York and use this form for New York taxes.

The person who may speak for you

A person, not a firm. Example: Alex Counsel. Do not write SingleFax.

First and last name. Example: Alex Counsel.
Example: 200 State Street.
Example: Albany.
Their state, which can be outside New York. Example: NY, or FL for a Florida attorney.
Example: 12207.
Example: 518-555-0199.
Example: New York CPA. This is the title line, not the ID number.
The short box next to the title. A PTIN such as P01234567, a Social Security number, or an EIN. Eleven characters at most.
New York’s preparer number. Leave blank if they do not have one. Eight characters at most.
Pick at least one. If this section is left blank, New York treats the form as covering every tax and every period.
Example: 2024. If the period is not a calendar year, write the start and end dates, such as 3/1/2024–2/28/2025. Do not write all open periods, current, or collection matters.
