Chase Fraud Fax Number: Debit, Credit, Auto and Home Lending
Find Chase’s published fraud document fax numbers by account type, prepare the requested paperwork, and keep a transmission receipt.

Chase publishes different fraud-document fax numbers for different account types. Report suspected fraud immediately using the number on your card or Chase’s official report-fraud page; then send documents to the unit handling your case.

ACH disputes need their own instructions
An unauthorized ACH withdrawal is not automatically a debit-card transaction. Tell the bank what appears on the statement, whether you recognize the company, and what you are disputing. Ask for the required authorization or return form, its deadline and the correct destination.
Use that form rather than adapting a credit-card dispute packet. A fax receipt cannot establish the type of transaction or extend a bank deadline. Keep a note of the date you reported the problem and the reference the bank assigned.
Prepare a readable fraud-document packet
Start with the form or document request Chase actually sent. Check that every required signature, date and transaction reference is present. Put the disputed items in an easy-to-read list if the bank requests one; distinguish transaction dates, posting dates and amounts rather than combining several charges into one total.
Include supporting material relevant to the request. For example, a cancellation confirmation may help explain a transaction you say should not have posted. Keep your originals and send legible copies. Do not add unrelated statements or pages containing other people’s information.
A cover sheet should identify the fraud department, your name and callback number, the case reference and total page count. Keep sensitive account information out of the cover unless the bank specifically needs it there. Passwords, card PINs and login codes have no place in the packet.
Send and track the documents

Save the exact file and transmission receipt alongside your case reference. Contact the case team to confirm receipt and ask whether anything is missing.
The delivery receipt proves the transmission result, not that Chase read the documents, accepted the dispute or issued a credit. Continue checking the bank’s case status through its official channels.
