There is no national DMV fax number. Each state has its own driver and vehicle agencies, and the right line can depend on whether you are submitting insurance proof, dealer paperwork, a disabled parking application, or documents requested after a license revocation.

This directory covers all 50 states and the District of Columbia. Fax numbers below were checked against the linked official pages on October 7, 2026. Re-check the official page the day you send. A number for one unit does not make fax an accepted channel for every DMV transaction.

Find your state and your document

The document column limits each verified fax to its published purpose. “No public fax listed on checked page” means that page did not publish one; other units may have their own numbers. “No public fax verified” means the official source could not be read successfully during this check. Use the official contact in either case.

For states with separate driver licensing and vehicle agencies, start with the agency responsible for your notice. Hawaii uses county offices; a state-level motor vehicle safety page helps you find the appropriate office.

Check these common DMV fax jobs carefully

Insurance proof: Pennsylvania’s Financial Responsibility line accepts updated insurance information. Use the reference number on your notice so the unit can match the documents to your record. Sending to an unrelated title or licensing fax may delay the response.

SR-22 certificates: An insurance card or policy summary is not necessarily an SR-22. Wisconsin’s instructions say the insurer normally files the certificate; if it will not, fax or email may be possible. The checked page does not publish a number, so ask WisDOT for the current destination before uploading anything.

Disabled parking and medical forms: Oregon’s application fax differs from its line for cancelling a deceased permit holder’s placard. North Carolina’s homebound ID service requires the request form and medical documentation. Washington separates non-CDL medical certificates from CDL medical waivers. Use the precise row and current form. The medical-document Send links open the HIPAA fax flow, with an electronic BAA before PHI upload.

Dealer and fleet documents: California’s Permanent Fleet Registration page directs Side A of MC 3500 P to its PFR fax, then describes later steps separately. Connecticut’s listed fax is for dealers adding a customer to online processing. South Carolina permits fax for dealer employee contact changes; its dealer renewal process is online. None of those lines is a general consumer registration service.

Colorado titles: The linked official ELT guide is dated 2015 and describes supporting documents from participating service providers for minor record changes. Confirm that workflow and the current destination with Title and Registration before sending. Do not use it as permission to fax an original title or a new registration application.

Virginia titles: We could not verify the current VSA-153 fax instructions from the live DMV source in this check. Use Virginia DMV’s official site and your current form; no prefilled fax is supplied here.

Send a DMV fax online

  1. Open the official source next to your state. Confirm the unit accepts your particular form by fax, including any required signatures and attachments.
  2. Use Send for the matching department, or open the send page and enter a number the agency gives you. Compare the prefilled digits with the current official instructions.
  3. Upload a clear, complete document. Include a cover sheet with your name, callback number, the unit name and the case or notice reference. Check every page, including the reverse of a two-sided form.

  5. Keep the document and delivery receipt. Ask the DMV whether it accepted the submission and whether fees, originals or further documents are still required.

A successful fax transmission does not restore a license, remove a suspension or complete a title transfer by itself. If a deadline is close, ask the unit how it determines timely receipt and which channel it recommends.

No. Driver and vehicle agencies are organized by state, and a published fax often serves only one department or form. Match both the state and the document before sending.
Only when your state and notice permit it. PennDOT publishes a Financial Responsibility fax for updated insurance information. That line does not accept insurance documents for another state.
Follow your state’s instructions and ask your insurer first. Wisconsin says an actual SR-22 certificate may be faxed or emailed if the insurer will not file it, but its checked page does not publish a fax number. Confirm the recipient with WisDOT.
